Are you looking for a new challenge and passionate about the area of Diabetes and Cardiology?
We have an exciting opportunity available for an Advanced Clinical Pharmacist to join our team and lead on these two specialty areas, supported by additional members of the pharmacy team.
You will lead on all aspects of medicines optimisation and medication use within the specialties, supporting medical and nursing teams, as well as allied health professionals in the safe and effective use of medicines.
Pharmacy and Medicines Optimisation at the Trust are well developed and we are committed to sharing excellence through our clinical services and teaching roles. We are actively seeking to progress and expand the roles undertaken by our senior clinical pharmacy team into areas such as out-patient clinics and patient support groups.
We will support you to continue to develop your advanced skills in clinical pharmacy, and would expect you to have a significant impact on the use of medicines by supporting safe and high quality services and supporting the patient journey.
Main duties of the job
Working within and leading a small pharmacy team, you will use appropriate skill mix within the team to ensure that medicines reconciliations are completed in a timely manner in line with current national recommendations, the safe and efficient use of patient’s own medication and effective communication through transfers of care.
Through excellent leadership skills, you will be expected to implement changes required to ensure that the service runs in a safe, efficient and cost-effective way.
You will develop excellent working relationships with senior leaders within these specialty areas, investigating opportunities for expanding roles to support patient pathways. You will work with senior pharmacy leaders to progress these opportunities and bring them to fruition.
