Job summary

An exciting opportunity has arisen to work with the dynamic West and North Hampshire Community Diabetes team as an experienced Diabetes Specialist Nurse, providing high quality diabetes care to patients in the community, and delivering diabetes training to health care professionals.

In this role, you will work as part of the multidisciplinary Diabetes Team providing specialist advice. You will be supported to demonstrate a sound knowledge of the issues related to the identification, assessment, treatment and management of this clinical group, particularly those with complex needs.

You will be supported to give specialist advice to GPs and other health professionals about diabetes, especially with reference to prescription and titration of medication.

You will be required to monitor and improve standards of care through supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.

Main duties of the job

  • Help support and manage people with diabetes referred to the service within a defined population as well as provide diabetes advice and guidance to other health care professionals as required.
  • Use clinical skills for assessing and treating patients with Diabetes and develop a plan of care along side them
  • Provide a point of contact for patient population offering specialist advice and referral routes
  • Communicate with service users referred into the service with respect and empathy, using person-centred approach and language and motivational interviewing skills to promote self-care and self-efficacy, referring to the document “Language matters” for appropriate language to use.
  • Monitor and review people with Type 1 or Type 2 diabetes in the Community via virtual clinics, telephone helplines, face-to-face clinics or group education with regard to symptom control, prescription and titration of medication adhering to local guidelines
  • Deliver diabetes care in clinic setting: face to face and remotely via; video, telephone and email following clinical guidelines and triage guidance
  • Be involved in delivery of structured group education for people with diabetes either face to face or via virtual platform for example Zoom
  • Plan and coordinate clinics with relevant colleagues to ensure that patients are seen in a timely manner in line with NICE Guidelines for Diabetes
  • Empower people with diabetes to make appropriate decisions about their treatment including adjustment of insulin dosage.
