Diabetes Specialist Nurse Development Role B6-B7
Job summary
A fantastic opportunity has arisen for a Diabetes Specialist Nurse to join our Cambridge Team working full time 37.5 hours per week. If you have compassion, empathy with the ability to work autonomously, this is the opportunity for you. The starting band for this post is band 6. Once started within the role you will develop an action plan with your line manager to support your transition into a band 7 role.
You will work as part of the multidisciplinary team with the aim of preventing admission and providing ongoing care for patients with Diabetes Disease within the community.
You will also be responsible for managing a caseload of patients with complex Diabetes needs, using evidence based practice and patient centred goals to assess plan implement and evaluate inventions
The role includes promoting better understanding of Diabetes for patients, their families and carers and other agencies involved with the patient through education, support and counselling.
Main duties of the job
- In conjunction with the multidisciplinary team and in line with local and nationaltargets provide specialist evidence based nursing advise to support and facilitatehigh quality, individualised care to patients/clients and relatives/ carers without supervision
- This will involve dealing with complex situations and having to take appropriate action in a variety of settings
- Provide expert advice and education for Healthcare professionals involved in the care of people with diabetes and their carers.
- Acting as a resource for general practice and working to reduce admissions, improve diabetes outcomes and quality of life for people with diabetes
- To provide advanced clinical practice in the field of diabetes care and ensure the provision of a quality patient focussed diabetes service in Cambridgeshire and Peterborough NHS Foundation Trust.
