Job summary
This role is primarily to lead the Community diabetes service though recovery and transition, by supporting GP practice staff to improve routine diabetes care, leading to the achievement of the 8 care processes delivered in primary care and to support the completion of annual reviews.
To provide and shape a service which enables general practices to improve outcomes for people with Diabetes within the Vale of York.
As the Lead Community Nurse for Diabetes, you will support and manage the Community Diabetes specialist nursing team working in collaboration with the Lead Diabetes Nurse, offering specialist knowledge, support, advice and education in relation specifically to Diabetes, primarily to health care professionals but also to those living with Diabetes and their carers’, where appropriate the post holder will be required to visit practices throughout the Vale of York geographical area.
Main duties of the job
The post holder will carry continuing responsibility for a defined caseload which will initially be comprised of practices achieving less than 50% of their annual diabetes reviews. The post holder will be required to be a non-medical prescriber.
The jobholder will play an essential role in assisting in the delivery and ongoing development of the diabetes service within primary care. They will work jointly with the medical and nursing team, and other health care professionals such as the dieticians and podiatrists.
The job holder will be professionally accountable to theDiabetes and Endocrinology Lead Nurse and will also be accountable and work collaboratively with the Lead Clinician for Diabetes and Endocrinology, regarding service development.
As an experienced nurse, the jobholder will ensure effective communication is maintained at all times, providing individual patient care and education to patients. He/she will play a key role in progressing with innovation care delivery in primary care and contribute to outreach services in community settings. The post holder will be expected to support the endocrine and diabetes team in the undertaking of high-quality audit and research designed to evaluate and improve the service.
There will be a focus on supporting educational needs in primary care, leading and developing education programmes to support ongoing training and development for primary care staff.
