Job summary

The Diabetes Team at Worcestershire Acute Hospitals Diabetes arerecruiting an experienced Diabetes Specialist Nurse to join our team. The post will be based at the Alexandra Hospital in Redditch and cover the Redditch and Bromsgrove locality. As well as being an experienced DSN you will also be required to be the team lead for local the service. You will be supported in this by a Lead Nurse for Diabetes and your colleagues locally and across the wider Diabetes Service. Community DSN services are provided on and off site. Ideally we are looking to recruit 1 postholder to a fulltime post but will consider jobs hare or part time hours for the right candidate(s).

You will bring a wealth of diabetes experience to the team in all aspects of diabetes care including management of Type 2 and Type 1 Diabetes, including insulin pumps / hybrid closed loop systems.

The team provides diabetes care directly to people living with diabetes and supports the wider team linking with our Diabetes consultants GP practices, neighbourhood teams and other care providers.

The community diabetes service also delivers diabetes structured education, DAFNE, Xpert and XPERT insulin as well as other diabetes education sessions and facilitating peer support sessions. Education and support extends to Primary Care teams and others, ensuring opportunities to provide good diabetes care enabled wherever it is needed.

Main duties of the job

Receive referrals into the service assess and effectively prioritise these in order to autonomously manage your caseload. Work both autonomously and as part of the team to provide a resource of highly specialist advice, information and support to all members of the primary health care team in the delivery of effective diabetes care. Attend diabetes care MDT meetings, presenting new and complex patients in line with clinical governance.

Undertake non-medical prescribing in line with trust guidance. Ensure you keep up to date with regular training and updates. Maintain a programme of professional development by attending in-house and external training, education, conferences and seminars in line with PDR/KSF review. Undertake mandatory training and keep this updated in a timely way. Continuously develop your highly specialist clinical knowledge, skills and competence through networking with other specialists and professional organisations. In collaboration with others develop guidelines and policies for use inside and outside the organisation.

Manage your team on a day to day basis including rosters, annual leave and personal development reviews. Informing the Lead Nurse promptly of any issues that arise.

Participate in audit and research.

Communicate with your team and with other healthcare professionals to ensue patient safety.
